    Aims and Objectives: In response to the SSI report ‘From Dependence to Independence’ (2000), Homefirst Community Trust (now part of NHSCT) and the NHSSB commissioned a Transition Project to consult young people, families and professionals/service providers.The unique multi-disciplinary Transition Team was established in September 2004 to address the recommendations of the project in order to smooth the transition to adult life. Aims: Engage with young people using a person-centred approach to support them to prepare for, and move into adult life. Enhance the quality of life for young people with physical and/or sensory disabilities by promoting meaningful inclusion, independence, choice, responsibility and participation.
